Leave of Absence

Students must attend classes every quarter (except Summer) in order to maintain active status in the Healthcare Informatics program. Current students may request up to two quarters off by completing this Leave of Absence form. Newly accepted students may defer their acceptance up to two quarters by using this form as well.

  • Please note that this request is only good for one quarter of leave at a time. Please submit a separate form to continue your leave of absence for up to two consecutive quarters.
  • Please provide your personal email as your BC email may not always be the best way to reach you while not in the program.
  • Please give a reason for your Leave of Absence request. You do not need to give details however the more you are willing to share, the better the Healthcare Informatics program is able to make their decision and assist you.
