Reinstatement Application :: Healthcare Management & Leadership Forms

Welcome to the reinstatement application for the Healthcare Management & Leadership program. This form is for students who were previously admitted into the program and wish to resume their studies following a formal dismissal. You will be required to meet the eligibility requirements in place for the most current application and supply new supporting materials. You will be charged the most current application fee and be placed on the most up to date degree worksheet. The decision to readmit a student who has been dismissed from HCML is determined by the Program Chair, who may consult with program faculty. Submitting an application to reinstatement does not guarantee readmission to the program.

Two (2) letters of recommendation are also required. Letters of recommendation must be on company letterhead from someone who personally knows your work. If you are employed, at least one letter must be from a current, direct supervisor discussing your contributions to your work place and how they believe you will benefit from completion of the Healthcare Management & Leadership BAS program. The direct supervisor’s letter must include your dates of employment, description of duties, and the supervisor’s title and contact information. Letters can also come from a current or former college instructor, on college letterhead and including the author’s contact information.

- I understand that if I am reinstated, I am required to complete all coursework with a “C” grade (2.0) or higher. If I fail to do so, I will be dismissed and will not be reinstated to the program.
- If I am reinstated I understand that I will be enrolled on a probationary status.
- I understand that submitting this application for reinstatement does not guarantee my readmission into the program.
