To join Neurodiversity Navigators, there are some documents you will need to send us prior to your intake appointment. Your intake appointment is where we make a final determination together regarding how Neurodiversity Navigators will fit your needs.
Here are the documents we need:
- Your unofficial high school or college transcript. We are simply looking at these to support you in choosing classes and determining what you need to do next!
On the form below (scroll down!) upload one of the following documents: (only one needed!)
- Unofficial High School transcript
- Unofficial College transcript, showing completed or attempted English course
- Smarter Balanced test scores
- AP English class or test scores
- AP/IB/Cambridge test scores
- ACT/PSAT/SAT scores
- Accuplacer assessment scores from any college within the past 4 years,
- GED Language Arts scores
- Also upload your NdN New Student checklist [PDF]. Be sure to complete all items on this checklist prior to requesting an intake meeting with us.
- If there is anyone else coming to the meeting with you, be sure to fill out a Release of Information Form to record what information you would like to share and who you would like to share it with prior to the meeting.
