To join Neurodiversity Navigators, there are some documents you will need to send us prior to your intake appointment. Your intake appointment is where we make a final determination together regarding how Neurodiversity Navigators will fit your needs.
Here are the documents we need:
- Any of the documents listed below. We are looking at these to support you in choosing classes and determining what you need to do next!
On the form below (scroll down!) upload one of the following documents: (only one needed!)
- Unofficial High School transcript
- Unofficial College transcript, showing completed or attempted English course
- Smarter Balanced test scores
- AP English class or test scores
- AP/IB/Cambridge test scores
- ACT/PSAT/SAT scores
- Accuplacer assessment scores from any college within the past 4 years,
- GED Language Arts scores
- You will need to check off boxes on the form indicating what you understand about the Neurodiversity Navigators program. You should ask a question or give us further information for any box that you do not check. You can provide this on the form.
- If there is anyone else you would like us to be able to speak with, be sure to fill out a Release of Information Form to record what information you would like to share and who you would like to share it with.
