Yoga Alliance Certificate Request Form

Please read the following instructions carefully. You must have a scanned copy of your Bellevue College Certificate ready to submit.

This form is to be used by students who have completed the Yoga Instructor Certificate program at Bellevue College and would like to request their Yoga Alliance Certificate.

Once a student completes the program and applies for graduation, students will be receiving two separate certificates. The first certificate is the Bellevue College Certificate issued to you by the college and shows your completion of the Yoga Instructor Certificate Program. The second certificate is the Yoga Alliance Certificate that is issued by our department and you will use this to register with the Yoga Alliance.

Students must receive their Bellevue College Certificate before requesting a Yoga Alliance Certificate. You will need to attach a copy of the Bellevue College Certificate as part of your request below.

Students must receive a copy of their BC Certificate of completion of the Yoga Instructor Program prior to obtaining their Yoga Alliance Certificate.
