Other Common App forms (or similar) requesting midterm grades (such as \u201cMidterm Report\u201d and \u201cFinal Year Report\u201d), high school information, or counselor statement CANNOT<\/strong> be completed by the Enrollment Services\/Registrar’s Office and should NOT<\/strong> be included when this form is submitted. The student is responsible for contacting the appropriate individuals\/departments for completion of those forms. Not all schools require these other forms. Check with each university on your list on their procedures.<\/p>\n\n\n\n Process Instructions:<\/strong><\/p>\n\n\n\n 1. Download the College Report from your Common App portal or the college\/university\/transfer application’s website. 3.
