Students cannot submit this form. This form is for faculty only.

  1. Work with the student to determine if a Course Challenge exam or Prior Experiential Learning portfolio is appropriate.
  2. Submit this form, and select the “Initial Request” option.
    • Indicate which courses you will review for credit.
  3. The student will be charged according to the total credit value.
  4. Confirm the student has paid the fees.
  5. Proceed with your portfolio review, or issue the challenge exam.
  6. Submit this form again with the final results.
