This form should be used to request completion of reports required for transfer admission such as the Common App College Report, Coalition Application Transfer Report, Deans Report, SCOIR, other college/university’s Transfer College Report, etc.
Other Common App forms (or similar) requesting midterm grades (such as “Midterm Report” and “Final Year Report”), high school information, or counselor statement CANNOT be completed by the Enrollment Services/Registrar’s Office and should NOT be included when this form is submitted. The student is responsible for contacting the appropriate individuals/departments for completion of those forms. Not all schools require these other forms. Check with each university on your list on their procedures.
Process Instructions:
1. Download the College Report from your Common App portal or the college/university/transfer application’s website.
2. Complete any required “Applicant” or “Student Section” portions of the form, and sign in designated applicant signature area.
3. Submit the completed and signed form using the upload button under Step 2 of the College Transfer Report Request Form below. Uploads must be in PDF format.
4. Provide the email addresses for the transfer institutions to which the College Report should be sent.
PLEASE NOTE:
Requests typically take 3-5 business days for processing. Be sure that you are aware of your application deadlines, and submit your request early, as there is no option for expedited service.
